Male Sexual Health

What is Premature Ejaculation?

Premature ejaculation is the most common male sexual dysfunction. This is defined as ejaculation in 1 minute following sexual intercourse. If untreated, this problem can cause significant psychological problems. This issue also can lead to self-confidence problems, unhappiness and depression for both men and their partners, as well as divorce in marriages.

How Does Ejaculation Occur?

Before defining premature ejaculation, normal ejaculation and ejaculation duration must be defined. Ejaculation in men almost always follows orgasm. The semen released from the testicles, prostate and vesicula seminalis with the help of nerves from the brain and spinal cord is ejaculated via the penis. Meanwhile, certain muscles contract at the end of the penis and help sperm to be ejaculated outside the body. This mechanism is controlled by both psychological and neurologic systems.

How Long Does Ejaculation Take?

It is hard to identify ejaculation time for men and to give them a normal time. Using a chronometer to measure ejaculation time has negative impacts on ejaculation and leads to stress. Although previous studies reported the global average as 5 minutes, the expectations of both the individual and the partner must be met for a healthy sexual life. Therefore, it is important to remember that everyone has an ejaculation time of their own.

What are the Reasons for Early Ejaculation?

Although the most common reason for early ejaculation is psychological, there are multiple reasons.

  • Performance anxiety (This might cause stress; the individual cannot escape this cycle),
  • Erectile dysfunction,
  • Sexually transmitted diseases,
  • Prostate inflammation and prostate disease,
  • Incorrect circumcision,
  • Depression,
  • Thyroid diseases.

Causes of Premature Ejaculation

Does Frequent Masturbation and Watching Pornography Cause Premature Ejaculation?

Control over one’s ejaculation can be similar to riding a bike. Ejaculation time and behaviour have been learned since adolescence and continue throughout life. Therefore, masturbating in a short time with the fear of being caught by parents lays the foundation for premature ejaculation problems in sexual intercourse.

Additionally, traumatic relationships cause permanent negative effects. In addition, exaggerated sexual experiences with pornography can lead to anxiety. Therefore, it is important to know that the duration and actions in pornography content are not realistic.

At this point, parents should provide a free environment for their children to help them have a healthy sexual life and take precautions to lay the foundation for a healthy sexual life.

What are Premature Ejaculation Types?

Although there are many sub-groups for premature ejaculation types, there are 2 main groups.

  1. Primary (lifelong) premature ejaculation: There is premature ejaculation in almost all relationships with the first sexual experience. Ejaculation’s duration is generally less than one minute. The underlying organic reasons must be investigated.
  2. Secondary (acquired) premature ejaculation: Individuals might experience premature ejaculation problems in different periods. Secondary premature ejaculation is a sub-group that can be experienced during certain periods of sexual life. In general, this can be treated more easily than primary premature ejaculation.

How is Premature Ejaculation Diagnosed?

The patient’s medical history plays an important role in the diagnosis of premature ejaculation. Patients should be able to share their sexual experiences openly with their doctors. Their medical history, medication and additional diseases must be investigated. Certain blood tests might be requested to eliminate certain diseases.

Below are 3 symptoms that are important to a diagnosis of premature ejaculation:

  • If the individual is ejaculating in the first minute before and after sexual penetration in almost all attempts.
  • If the individual ejaculates without delaying or controlling.
  • If the individual refrains from the sexual relationship due to chronic failure sensation and feels depressed.

Does Circumcision Cause Premature Ejaculation?

Scientific studies have shown that circumcision does not cause premature ejaculation. However, an incorrect circumcision might increase penile sensitivity and cause premature ejaculation.

Can Ejaculation be Controlled?

Ejaculation is a controllable, complex process. Individuals can learn to control their ejaculation duration. For this purpose, a condom or gel can be used to decrease penile sensitivity and control ejaculation. However, the most important control can be achieved by mentally controlling ejaculation time. Special devices and mobile applications have been developed for this purpose. This way, it is possible to have longer ejaculations and control ejaculation.

Premature Ejaculation Treatment

The first step in premature ejaculation treatment is behavioural treatment. It is important to decrease the patient’s anxiety and eliminate the organic cause. Exercising, maintaining a healthy weight, regular and sufficient eating habits and performing the exercises recommended by the doctor will have positive effects on ejaculation time and control.

However, patients who do not benefit from these treatments might use certain medications. If the medication does not help, fillers to the glans penis are the most common alternative. Botox injections in the ejaculation muscles are also used as an effective treatment for premature ejaculation.

Are Premature Ejaculation Treatment Costs High?

Premature ejaculation is an important sexual dysfunction that causes unhappy sexual lives for both men and women. Many men and their partners are influenced since this problem is prevalent in society. Successful results can be achieved if there is patience with premature ejaculation treatment. Although the treatment has different prices among hospitals and clinics, the cost is not high.
